Information You have the Right to Know and Discuss with Your Treatment Team

• What is my diagnosis and what does it mean?
• What types of treatment are recommended for my illness?
• What are the expected benefits and risks or negative side effects of any recommended treatment?
• About how long will I be expected to be in treatment?
• What should I do if I feel the treatment isn't working?
• If I don't like the treatment? If I want to change clinicians or psychiatrists?
• What happens if I am in crisis and need to be hospitalized?
• How can I learn to manage my illness?
• Who do I talk to if I have a complaint about my services or one of my providers?
• What is the Center's philosophy regarding recovery and empowerment?
• What are my rights as a consumer of mental health services, and what specific rights do I have if I am a Medicaid recipient?

If you have a Complaint:

Complaint procedures are posted in each office. You may request complaint procedures from your clinician, Jefferson Center Staff or the Consumer and Family Advocate for Jefferson Center.
