Consent to Receive SMS Text Messages
We value your privacy and want to ensure that you are fully informed about the types of communications you may receive from us. By providing your mobile number and selecting SMS text messaging as an authorized method of contact, you give Jefferson Center for Mental Health (“Jefferson Center”) prior express consent to send SMS text messages to the phone number you have provided, including messages sent by automated technology where permitted by law. Your consent applies only to messages from Jefferson Center and does not authorize unrelated marketing, promotional messages, or messages from third parties. Consent to receive text messages is voluntary and is not a condition of receiving treatment, services, or benefits from Jefferson Center.
Because Jefferson Center is a health care provider, some SMS text messages may include information related to your care, appointments, billing, treatment, or health services. Text messaging is not intended for emergencies, crisis situations, urgent clinical needs, medication changes, or time sensitive clinical instructions unless specifically directed by your care team. If you are experiencing an emergency, please call 988 or visit Jefferson Center’s walk-in-crisis center at 4643 Wadsworth Blvd, Wheat Ridge, Colorado 80033.

Terms and Conditions:
- Voluntary Consent: Your consent to receive SMS text messages is voluntary. You may refuse or withdraw consent at any time, and your decision will not affect your ability to obtain treatment or services from Jefferson Center.
- Automated Technology: By selecting SMS text messaging as an authorized method of contact, you consent to receive text messages that may be sent using automated technology where permitted by law.
- Message Frequency: SMS text messages may be recurring. Message frequency may vary depending on your appointments, care, billing, services, and communication preferences.

- Privacy: We will protect your personal information in accordance with our Privacy Policy and HIPAA Notice of Privacy Practices. Your phone number will not be shared with third parties for marketing purposes.
We do not guarantee the successful delivery of text messages by your wireless provider. Messages sent by text may not be delivered if the mobile device is not in range of a transmission site, or if the network is down. Factors beyond the power of wireless carriers may get in the way of message delivery. This may include the terrain, how close you are to buildings, foliage, weather, and your equipment. We and your wireless provider will not be liable for losses or damages that come from:
- a message not delivered, a message delivered late, or
- a message that goes to the wrong number, or
- inaccurate or incomplete content in a text message.
We are not liable for your use or reliance on the content of any text message.
If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
Privacy & Security
To the extent you provide a mobile number that is later reassigned to another person or you no longer use that number, you agree to notify us promptly so we can update your communication preferences. Jefferson Center will rely on the mobile number and consent information in your health record unless and until you update it or withdraw consent.
We recommend that you use a password, passcode, biometric lock, or other security feature to open and protect your mobile device. Text messages may include protected health information (PHI). Because standard SMS text messaging may not be encrypted, there is a risk that PHI could be intercepted, viewed, stored, forwarded, or disclosed to others, including anyone with access to your device, phone account, shared phone plan, message notifications, cloud backups, or wireless carrier systems. By choosing to receive text messages from us, you acknowledge and accept these risks. The use and disclosure of PHI in text messaging may also be addressed in our HIPAA Notice of Privacy Practices.
Jefferson Center will make reasonable efforts to limit the amount and sensitivtiy of PHI included in SMS text messages and to use text messaging in a manner consistent with applicable privacy and security requirements.
Privacy Policy Notice: Please refer to our Privacy Policy and our HIPAA Notice of Privacy Practices for more details on how we protect, use and disclose your personal information and protected health information.
