Your care team includes therapists, care coordinators (case managers), peer support specialists, prescribers, and nurses.
- Therapist: helps you learn and gain skills and tools that will help you to feel better
- Care Coordinator: helps you connect to resources that impact your mental health through Jefferson Center and other community resources
- Peer Specialist: helps you connect to someone who has lived experience dealing with mental health issues
- Prescriber: helps you with medication management and biomedical needs related to mental health
- Nurse: assists you and the prescriber with medication management

These Collaborative Care Clinics are new and exciting! With each new things comes questions, and we’ve tried to think of anything you might ask. If you don’t find the answers you are looking for below, give us a call and we can help.
Can I keep my current prescriber, therapist, case manager, or peer?
If you choose to be part of the clinic, you will be asked to transfer your care to the clinic providers for those specific services.
If I am interested in only receiving medication services, can I receive my services in this clinic?
It depends on your individual needs and preferences. A provider will work with you to co-create a plan of action for your care.
How often can I visit the clinic?
You can come at any time the clinic is open and can receive services based on availability. However, you may not receive duplicate services in one day (ex. you cannot have two therapy appointments in one day)
Will I be able to receive clinic services outside of clinic hours? (emergency med refill, case management, peer support, etc?)
No, services are only available during posted clinical hours. However, if you are experiencing a crisis we encourage you to call crisis services. You can always call or email our staff and we will respond within a week.
If I receive psych meds from my PCP outside of Jefferson Center, can I continue with non-med clinic services?
Yes, if you have completed an intake with Jefferson Center and are eligible services, you can receive a variety of non-medical services from the Collaborate Care Clinic.
If I am receiving therapy from outside the clinic, can I come to the clinic?
Jefferson center clients that are receiving outside therapy and only want med services, might be a better fit for the Mostly Meds program. Collaborative Care Clinics ask that clients engage with the team in other aspects of the clinic in addition to medical services.
If I receive a case management referral to navigation or a community partner, how would I make sure I am not double billed for that service?
The Clinic Care Coordinator will assess with you where you are receiving services from, collaborative with the other provider as needed, and make recommendations for who you should see for specific case management services inside and outside the clinic.
Are Employment Specialists and Wellness Coaches able to join the clinic's zoom to meet with me or will I have to meet with other Jefferson Center services outside of clinic?
For now, you will have to meet with Wellness and Employment Specialists outside of the clinic. We still would love to collaborate with those other services not offered in clinic and will keep in contact with them regarding your care.
What criteria is there to receive these services?
The initial qualification for the clinic is to have Medicaid. Specific clinics may have other qualifications based on location or scope of services.
What is the typical wait time?
Wait time varies and is based on how busy the clinic is at the time of arrival and what services are needed that day.
Will I be able to make an appointment with a prescriber or will it be completely on a walk-in basis?
If clients are new to the prescriber and new to medication services, an initial medication evaluation will be scheduled since these are typically longer appointments. At this time all follow up services will be walk in only.
