STEP 1: Please carefully read these detailed instructions and the first few pages of the Credentials Application for information on completing your Credentialing application. Please also take a minute to review the Credentialing Policy and Procedures.
STEP 2: Save the following files on your computer.
Important: These are very large files. It will take a minute to download completely. While it is downloading you will not be able to enter data into the fields, and you should wait until it is downloaded to save it to your computer. When it is finished downloading you will have an “I” cursor and be able to type in the Last Name field.
