Jeff Plaza Services Moving to New Locations

Dear Jefferson Center Client,

Your care and support are always our top priority. With that in mind, we want to share an upcoming location change and how we will work with you to ensure your care continues without disruption.

Beginning March 31, 2026, we will no longer be offering services at our Jeff Plaza (3595 S Teller St, Lakewood, CO 80235) building, and will be moving all in-person appointments at this location to our Alameda (5801 W Alameda Ave, Lakewood, CO 80226) or Union (12055 W 2nd Pl, Lakewood, CO 80228) offices . We are partnering with your care team to identify your new care location. We will send an update as soon as possible with your new location.
