Welcome to Jefferson Center’s Recovery Teams. Our Recovery Teams are multidisciplinary teams comprised of Certified Addiction Counselors, Licensed Professional Counselors, an Intake Clinician, a Care Manager and a Prescriber.
The Recovery Teams are a support system to assist you in becoming engaged in a collaborative treatment process designed to manage your mental health and substance use conditions more effectively and to increase access to the services you may need. Because substance use and co-occurring mental health disorders come with a complex set of circumstances and treatment needs, the Recovery Teams recognize the need for continued and complex treatment and support for long-lasting recovery.
In this team approach, each client will be assigned a Care Manager who will assist in connecting you to a range of activities intended to improve patient care.
Your treatment Team team consists of your therapist and your care manager. While your initial appointment will be with one of these therapist, you can see others on your team to reduce access time and/or quickly see a member of this team during a crisis.
We are pleased to offer a wide range of services/resources, including:
- Immediate access to individual and group services
- Individualized treatment plans, tailored to meet your unique needs
- Medication evaluation/medication management
